The License Clinical Social Worker will be an integral member of the NHCLV multidisciplinary team to provide mental health treatment for patients in the Integrated Behavioral Health program at NHCLV. The License Clinical Social Worker educates patients about mental illness through the use of psychoeducation and provides mental health counseling within individual, family, couples, and group counseling settings. The License Clinical Social Worker works closely with the primary care provider, behavioral health integrated specialist, and a consulting psychiatrist to revise the wellness plan when patients are not improving, and offer alternatives as needed to support treatment. The License Clinical Social Worker will evaluate, assess, and support the patient in the office, their home, and community as needed. The License Clinical Social Worker will utilize patient centered, family focused therapy using a variety of treatment modalities such as Cognitive Behavioral Therapy and Solution Focused therapy.
MSW Care Manager is responsible for providing and coordinating of mental health care for patients at NHCLV. The MSW Care Manager educates patients about the intersection of behavioral health, mental illness and their treatment, provides behavioral activation, and monitors depressive symptoms and response to medication and/or psychotherapy using a structured instrument (e.g., the PHQ-9). MSW Care Manager works closely with the primary care provider, mental health providers, drug and alcohol counseling partners, and a consulting psychiatrist to revise the wellness plan when patients are not improving, and offer alternatives to support treatment.
RN Clinical Nurse:
The Clinical Nurse II (RN) will be an integral member of the health center care team. Together with providers, nurses, medical assistants, patient navigators, behavioral health specialists, and outreach staff, the RN will provide high quality, cost effective medical care essential to the assessment, promotion, maintenance and restoration of patients’ health and wellness. The RN will assist with day-to-day operations in the health center, focusing on appropriate access to care. The RN will also participate in care plan implementation and management for assigned populations, ensure continuity of care and assist with care coordination, and serve as a clinical resource for other health center staff. The RN will work under the direction of the RN Clinical Coordinator.
The Clinical Nurse I (LPN) will be an integral member of the health center care team. Together with providers, nurses, medical assistants, patient service representatives, behavioral health specialists, and outreach staff, the LPN will assist with day to day operations in the health center, participate in care plan implementation and management for assigned populations, ensure continuity of care and assist with care coordination, and serve as a clinical resource for other health center staff. The LPN will work under the direction of the Nursing Services Manager/RN Clinical Coordinator.
Nursing Services Manager:
The Nursing Services Manager promotes the delivery of high-quality team-based patient care through a cross functional, interdisciplinary approach consistent with the concepts of Patient Centered Medical Home and all aspects of the Quadruple Aim. This role is responsible for leadership, support and guidance to all nursing and clinical support staff. The Nursing Services Manager will serve as a clinical resource for health center team members.
RN Clinical Coordinator:
The Clinical Coordinator will be responsible for planning, coordinating and evaluating daily operations and serving as lead for the clinical team. The Clinical Coordinator will be responsible for orienting all new clinical staff to the practice and clinical policies and procedures. The incumbent will assure that excellent patient/family centered planning, communication and care is provided to each patient. They will use collaboration, coordination, delegation and supervision in working with the health care team to achieve evidence based care and optimize patient care outcomes. The incumbent will serve as a clinical resource for clinical staff, and clerical staff, including Practice Manager.
The Controller will be responsible for providing strategic and fiscal leadership by working with the Senior Leadership Team to establish long-range goals, strategies, plans and policies. Responsible for the oversight of all fiscal operations, including general accounting, budgeting, payroll, accounts payable, fee for service billing and patient accounting, managed care claims processing, grants administration and other contracted services billing and collection.
This position is responsible for routine accounting and financial activities.
The COE Behavioral Health Integrated Specialist is responsible for assessing interested patients, designing care coordination plans and coordinating care management for Center of Excellence patients at NHCLV. The COE Behavioral Health Integrated Specialist is based primarily in the primary care clinic, and provides screening, assessment, evaluation of needs, care management plan development and linkage to higher levels of care for patients interested in recovery support for substance use disorder. Works with a multidisciplinary care management team comprised of clinicians, community health workers and drug and alcohol counseling partner agencies to support patients on their path to recovery.
To provide case management services to patients with identified needs related to psychosocial needs and medical services. Have the ability to work in collaboration with medical teams on cases. Serve as an advocate and liaison for and on behalf of patients’ rights and benefits associated with Compass/Medicaid applications, Affordable Care Act, prescription assistance applications, and additional medical plans or psychosocial resources as well.
