The following health care services are offered to NHCLV patients. If you are interested in becoming a patient at one of our community health centers in the Lehigh Valley, please contact us.
Behavioral Health/Mental Health Counseling
Screening, diagnosis, and counseling for mental health conditions such as depression or anxiety, or referral to another community mental health provider who can meet your needs. Our behavioral health staff work with your primary care provider to ensure your care plan is coordinated and takes your needs and preferences into account. We work with individuals, couples, and families, and we see children as well as adults for counseling. Group counseling is also offered.
Breastfeeding (Lactation) Services
Certified nurses and other staff will assist new moms and their babies with starting and continuing breastfeeding. We can provide education that includes normal newborn feeding patters, how to tell if your baby is getting enough to eat, and how to use a breast pump.
Case Management & Social Services
Assistance with applications for insurances and public benefits, prescription assistance, rental and food bank assistance, social security benefits, Health Insurance Marketplace (Affordable Care Act) applications, transportation, or other programs that you may qualify for.
Group well-child visits with babies that are a similar age that include education and support for moms and families. You will learn information to help you make healthy choices for your child as well as your whole family. All of your child’s visits from age 1 month up to 2 years will occur with the same group of families.
Prenatal visits with a group of other pregnant women and your healthcare team that include prenatal care and additional time for learning and support. You will have more time with your provider, learn more about pregnancy and birth, and be better able to care for your new baby. All of your prenatal visits occur with the same group of women.
Chronic Care Management
If you have health conditions such as hypertension, diabetes, or heart disease, our team of nurses will assist you to set and achieve goals that you want to reach. We will provide support and education so that you understand your care plan and will make sure we understand your preferences so we can partner with you to improve your health.
COVID-19 Testing and Vaccinations
COVID-19 testing is offered for NHCLV patients ONLY as follows:
Mondays – SouthSide Bethlehem / Wednesdays – Downtown Allentown / Fridays – Easton
Patients must first have a telemedicine/virtual visit with the NHCLV care team before coming in for the test. (NOTE: Walk-in testing is NOT available.) Please call 610-841-8400 to schedule an appointment.
COVID-19 Vaccinations are coming soon at NHCLV and will initially be available only for people meeting the Phase 1A criteria as determined by the Pennsylvania Department of Health. Check back for updates as they become available.
Preventive dental services such as basic dental screening, cleanings, and oral health education are offered in our Easton location.
Diabetes Management Classes
If you or a family member have type 2 diabetes, join this six-week program to learn strategies to lower your blood sugar and keep it under control with diet, exercise, and stress management. A trained health educator will guide you along the way and you will receive support from other participants who have the same goals and challenges as you.
We offer on-site pregnancy testing, counseling on birth control options, and procedures for certain methods such as intrauterine devices (IUDs) or implants (such as Nexplanon). If you do not have insurance, we can work with you to find an affordable option.
Medication Assisted Therapy for Substance Use Disorder
Screening, diagnosis and treatment for substance use disorders including opioids and alcohol. If our services are not right for you, we’ll help you find treatment with another organization in the region.
We provide all primary care services your child needs, including well-child visits, immunizations, sick visits, screenings for vision and hearing, school physicals, and referrals if your child needs specialist care. In addition we do screening tests for developmental issues and mental health problems. We can work with your child or refer them to other community providers for management of mental health and developmental problems.
Learn how you can prevent Type 2 Diabetes by making simple lifestyle changes. This program is approved by the Centers for Disease Control and is proven to cut your risk of getting diabetes by half. A trained lifestyle coach will guide you along the way and you will receive support from other participants who have the same goals and challenges as you.
Prenatal & Postpartum Care
Family-centered care for the expecting mother and her baby. We provide recommended screenings like lab tests, ongoing check-ups throughout your pregnancy, and counseling and education regarding childbirth, nutrition, breastfeeding, and postpartum care. If your pregnancy is high-risk we will assist you to find an OB and follow up with you to make sure you are receiving the services you need. At this time we do not do ultrasounds in our health center, but we will help you coordinate any need ultrasounds. For deliveries, we have a strong partnership with local hospitals and will coordinate all aspects of your care.
Primary Care Services
Medical care focused on prevention of health problems, as well as appointments if you are sick or to manage chronic conditions. Primary care services are for anyone—men, women, children and seniors. If we cannot take care of your health problem, we will refer you to a specialist and assist you with finding one. Depending on your age, family history, or other information we ask you, we will offer screenings for cancer, heart disease, or other health problems like Hepatitis C or other infections. We also offer all recommended vaccines for children, and certain age-appropriate vaccinations for adults.
Sexually Transmitted Diseases & HIV
If you think you have been exposed to a sexually transmitted disease or HIV, we provide testing, counseling and treatment. If you need follow up care that we cannot provide, we will help you to find a specialist that can take care of you, and we’ll make sure that you are receiving the services you need.
For your convenience, certain lab tests can be performed at our health centers. If we do not perform the lab test, we will work with you to identify a lab that takes your insurance or can offer you a discount on testing fees. At this time we do not offer any radiology tests at our health centers, such as X-rays, ultrasounds, or CT scans. We will assist you to find a testing center where you can have these tests done if recommended by your primary care provider.
We offer preventive care and treatment of certain gynecologic conditions, breast and pelvic exams, cervical cancer screening (pap smears), and certain gynecologic procedures. If a specialist referral is needed, we will help you find a specialist that can take care of your health condition.
